Board certification in Internal Medicine, Family Medicine, or Endocrinology is preferred, along with 2 years of experience and competence in diabetes management. Telemedicine experience is preferred.
The Continuity Clinician - Nurse Practitioner will provide safe and timely care during office hours through continuous remote care, collaborating with health coaches and other clinicians. They will proactively ensure adequate follow-up of patient concerns, share after-hours coverage with the team, actively participate in the development of new technology and protocols, and maintain patient confidentiality in accordance with HIPAA regulations. They will manage their panel of patients and deliver patient-centered care remotely.
