Consultative Pharmacist 2 - Hospice Palliative Care

Position Overview

Become a part of our caring community and help us put health first. The Consultative Pharmacist 2 consults with patients, pharmacies, and providers to provide patient care in the form of optimal medication therapy. This role provides end-of-life pain and symptom management and improves patient’s quality of life as a palliative care clinician for hospice patients. Work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

Schedule

  • Monday through Friday 2:30 PM – 11:00 PM EST (1:30 PM – 10:00 PM CST)
  • Includes a 4th weekend rotation

Location

  • Work-at-home opportunity based out of Pennsylvania or Tennessee.

Essential Duties and Responsibilities

Clinical Consultations

  • Recommends appropriate pain and symptom management therapies based on patient-specific information.
  • Provides real-time collaboration with nurses to create a cost-effective medication care plan.
  • Focuses on medication misadventure prevention by providing education to nurses and caregivers.

Enhancing Patient’s Quality of Life

Documentation

  • Advances patient care by documenting interventions for outcomes measures.
  • Maintains accurate patient profiles.
  • Verifies encounter information is complete before submitting for transcription/processing.

Quantity of Work

  • Produces a quantity of work that is at or above the documented average.

Professional Interaction

  • Possesses excellent communication and interpersonal skills.

Qualifications

  • Excellent communication skills.
  • Superior clinical assessment skills.
  • Strong commitment to patient care.
  • Proficient in general Microsoft Office.
  • Must meet minimum requirements for Humana’s Work-at-Home internet policy.

Education and/or Experience

  • PharmD or B.S. in Pharmacy from an accredited school of pharmacy required.
  • Experience with hospice care preferred.

Travel

  • While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Compensation

Pay Range

  • $94,900 - $130,500 per year
    • Note: The compensation range reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, certifications, etc.

Bonus Incentive Plan

  • This job is eligible for a bonus incentive plan based on company and/or individual performance.
