Requirements

Candidates should possess a Doctor of Pharmacy (PharmD) degree and be licensed as a Clinical Pharmacist in the state of California. They must have at least 3 years of experience as a Clinical Pharmacist, demonstrating expertise in medication therapy management and patient care. Strong knowledge of pharmaceutical principles, disease states, and medication interactions is essential, along with the ability to interpret and apply clinical guidelines and best practices.

Responsibilities

The Clinical Pharmacist will provide expert-level medication therapy consultation to patients and providers, collaborate with the Engineering team to improve the software platform, and supervise the Clinical Care support team. They will also maintain and review patient medication profiles, counsel patients on appropriate drug use and adherence, develop clinical recommendations, review medication reviews, and interact with other healthcare professionals to enhance patient outcomes. Additionally, the Clinical Pharmacist will assist with orienting new team members and Virtual Pharmacist users, facilitate prior authorizations, and support patients in accessing financial assistance and transportation.
