  • Must be willing to travel within a 90-mile radius from Wayne and Macomb counties at least 70% of the time

  • 1-3 years relevant experience (such as community outreach or home visits to members/patients)
  • High School Diploma or G.E.D
  • Knowledge of formal and informal community agencies and resources
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community
  • Ability to communicate medical information to health care professionals and care coordinators over the telephone
  • Ability to work collaboratively in a team, manage multiple priorities, utilize effective time management skills, and exercise sound judgment
  • Exceptional organizational and interpersonal skills, with attention to detail
  • Ability to effectively provide support care to socially and medically complex patients in a variety of non-traditional settings
  • Ability to influence medical care providers as needed to adjust interventions based on member’s cultural, ethnic, or religious preferences
  • Requires flexibility to work in situations in which the working conditions are unknown, as environments and situation of patient homes may vary
  • Experience serving the Medicaid Populations in urban or rural environments with familiarity of local, formal, and informal resource networks (preferred)
  • Community Health Worker Certification or equivalent training and experience (preferred)
  • Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions
  • Meet all requirements set forth by local state i.e. first aid, CPR, TB screen, etc

Responsibilities

  • Serve the community as a bridge between the member (community) and the health care system (providers) through outreach and education
  • Help Aetna members attain and maintain better health outcomes
  • Improve relationships with healthcare providers and help both parties become more acculturated with each other
  • Work under the direction of a designated professional
  • Work in a field-based position with members/community in assigned city/county and surrounding counties
  • Work scheduled hours M-F from 8:00am to 5:00pm (flexible), with first 6 weeks WFH training
  • Occasional weekend hours may be required
  • Travel to multiple office locations and home locations (up to 70%)
