Must be located in Michigan or surrounding area
Must be willing to travel within a 90-mile radius from Wayne and Macomb counties at least 70% of the time
Must possess reliable transportation
1-3 years relevant experience (such as community outreach or home visits to members/patients)
High School Diploma or G.E.D
Knowledge of formal and informal community agencies and resources
Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services
Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community
Ability to communicate medical information to health care professionals and care coordinators over the telephone
Ability to work collaboratively in a team, manage multiple priorities, utilize effective time management skills, and exercise sound judgment
Exceptional organizational and interpersonal skills, with attention to detail
Ability to effectively provide support care to socially and medically complex patients in a variety of non-traditional settings
Ability to influence medical care providers as needed to adjust interventions based on member’s cultural, ethnic, or religious preferences
Requires flexibility to work in situations in which the working conditions are unknown, as environments and situation of patient homes may vary
Experience serving the Medicaid Populations in urban or rural environments with familiarity of local, formal, and informal resource networks (preferred)
Community Health Worker Certification or equivalent training and experience (preferred)
Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions
Meet all requirements set forth by local state i.e. first aid, CPR, TB screen, etc

Responsibilities

Serve the community as a bridge between the member (community) and the health care system (providers) through outreach and education
Help Aetna members attain and maintain better health outcomes
Improve relationships with healthcare providers and help both parties become more acculturated with each other
Work under the direction of a designated professional
Work in a field-based position with members/community in assigned city/county and surrounding counties
Work scheduled hours M-F from 8:00am to 5:00pm (flexible), with first 6 weeks WFH training
Occasional weekend hours may be required
Travel to multiple office locations and home locations (up to 70%)
