Candidates must possess a Bachelor's degree in a related field with at least 3 years of professional experience, including 2 years of experience working with or in an FQHC and 2 years of progressive value-based care experience. They should be able to thrive in a fast-paced, startup environment, demonstrate experience taking an idea from concept to execution, and possess a proven ability to successfully manage multiple programs by coordinating and collaborating across projects and teams. Strong and clear thinking and writing skills are essential. Preferred qualifications include experience with Tableau or other analytics tools, change management experience (e.g., Lean Six Sigma), and a Master's degree or advanced degree in business, healthcare administration, or public policy. The role requires willingness to travel up to 50% of the time across US markets.
The Community Health Center Performance Management Specialist will analyze performance data, conduct workflow assessments, and identify improvement opportunities for CHC partners. They will collaborate with the COE team to develop targeted resources and processes to boost performance on critical metrics. This role involves facilitating COE meetings and events, coordinating cross-functional projects, and translating insights into actionable recommendations. Responsibilities include managing the pipeline of CHCs in different consulting phases, managing multiple COE events, developing solutions such as process maps and training materials, creating resources, and presenting information orally or in writing. The specialist will coordinate and collaborate with CHC Center of Excellence Staff on building program services and resources, and marketing this information. They will also coordinate consulting services with COE team members and other Aledade Field Staff on meeting logistics and follow-ups, and represent Aledade to CHCs during CHC summits and events. Additional duties may be assigned to ensure maximal coordination across Aledade teams.
