Requirements

  • Bachelor's degree in health and human services field
  • 2 or more years of related experience
  • Knowledge of Microsoft Office Word and Excel
  • Exceptional verbal/written communication and interpersonal skills
  • Ability to use a variety of electronic information applications/software programs
  • TB screening required if selected for the role
  • Valid state driver’s license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits
  • Must reside in Muncie IN or surrounding areas
  • Passionate about contributing to an organization focused on continuously improving consumer experiences
  • Preferred: Prior experience with Medicare & Medicaid recipients
  • Preferred: Experience with electronic case note documentation and multiple computer applications/systems
  • Preferred: Experience with health promotion, coaching and wellness
  • Preferred: Knowledge of community health and social service agencies and additional community resources

Responsibilities

  • Evaluate member's needs and requirements to achieve and/or maintain optimal wellness state
  • Guide members/families toward and facilitate interaction with resources appropriate for care and wellbeing
  • Coordinate services and resources
  • Identify and resolve barriers that hinder effective care
  • Ensure patient is progressing toward desired outcomes by monitoring care through assessment, data, conversations with member, and active care planning
  • Understand own work area professional concepts/standards, regulations, strategies and operating standards
  • Make decisions regarding own work approach/priorities, and follow direction
  • Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation
  • This role is patient facing and is part of Humana’s TB screening program (TB screening as part of the process)
  • Travel occasionally to Humana’s offices for training or meetings may be required
