Requirements

Bachelor's degree in health and human services field
2 or more years of related experience
Knowledge of Microsoft Office Word and Excel
Exceptional verbal/written communication and interpersonal skills
Ability to use a variety of electronic information applications/software programs
TB screening required if selected for the role
Valid state driver’s license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits
Must reside in Muncie IN or surrounding areas
Passionate about contributing to an organization focused on continuously improving consumer experiences
Preferred: Prior experience with Medicare & Medicaid recipients
Preferred: Experience with electronic case note documentation and multiple computer applications/systems
Preferred: Experience with health promotion, coaching and wellness
Preferred: Knowledge of community health and social service agencies and additional community resources

Responsibilities

Evaluate member's needs and requirements to achieve and/or maintain optimal wellness state
Guide members/families toward and facilitate interaction with resources appropriate for care and wellbeing
Coordinate services and resources
Identify and resolve barriers that hinder effective care
Ensure patient is progressing toward desired outcomes by monitoring care through assessment, data, conversations with member, and active care planning
Understand own work area professional concepts/standards, regulations, strategies and operating standards
Make decisions regarding own work approach/priorities, and follow direction
Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation
This role is patient facing and is part of Humana’s TB screening program (TB screening as part of the process)
Travel occasionally to Humana’s offices for training or meetings may be required
