Candidates must be fluent in Spanish and English, demonstrating strong communication skills. Prior experience in case management, particularly with individuals experiencing homelessness, Serious Mental Illness, or Substance Use Disorder is preferred. A Bachelor's degree in social work, psychology, or a related field is desirable.
The Lead Care Manager will directly outreach and engage with individuals living with Serious Mental Illness/Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs, building trusting relationships and coordinating comprehensive care. They will be responsible for participating in interdisciplinary care planning, conducting home visits, and advocating for clients’ needs. The role also involves collaborating with community partners and utilizing a proprietary data-driven technology platform for care coordination, while also participating in 1-2 on-site visits per month.
