Candidates should possess a Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree, and must be board-certified or board-eligible as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) with a focus on chronic disease management, behavioral health, and preventative care. They should have at least three years of experience in providing comprehensive clinical care to patients with chronic conditions, including hypertension, diabetes, and chronic kidney disease, and experience in managing behavioral health conditions such as depression, anxiety, and substance use disorders is preferred.
The Advanced Practice Clinician will conduct comprehensive clinical intake visits, provide longitudinal wrap-around care focusing on chronic condition management and preventative care, proactively outreach to members needing follow-up care, complete diagnosis capture related to HCC and ICD-10 coding, address contractual and company-prioritized quality gaps, prescribe medications for depression, anxiety, and substance use disorders, remain updated on evidence-based care for complex patients, refer members to specialists, support members in achieving care goals, participate in case conferences, and provide clinical input on care plans.
