Nurse Practitioner - Cardiometabolic Clinic

Position Overview

Devoted Health is seeking a compassionate and entrepreneurial Nurse Practitioner (APRN) with a passion for chronic disease management to join our next-generation virtual cardiometabolic clinic. This role is crucial in expanding access to medications for vulnerable seniors, focusing on optimizing medication adherence for members with hypertension, hyperlipidemia, and diabetes who are at risk of running out of their medications. You will serve as the prescriber in our "Medications on Time" service and collaborate with a clinical pharmacy team of over 90 pharmacy technicians. You will utilize and help improve our proprietary technology and electronic health information platform for virtual visits and asynchronous prescription refills.

Responsibilities

• Conduct focused and thorough assessments of patients with cardiometabolic conditions (hypertension, hyperlipidemia, coronary artery disease, diabetes) who are at risk of medication depletion.
  • Expect 40-60% of clinical effort to be dedicated to virtual visits.
• Formulate accurate diagnoses and develop individualized treatment plans, including medication management, lifestyle modifications, and monitoring recommendations.
• Manage a refill inbox for patients with cardiometabolic conditions according to established protocols and guidelines.
  • Expect 40-60% of clinical effort to be dedicated to managing a prescription refill queue.
• Identify and evaluate risk factors, comorbidities, and potential contraindications for treatment.
• Provide counseling on medication adherence and potential side effects.
• Collaborate closely with other care team members, including PCPs, endocrinologists, cardiologists, pharmacy, clinical nursing, social work, and interface with family members/caregivers to ensure holistic and coordinated care.
• Utilize our proprietary electronic health information system for visits and provide feedback for interface improvements.
• Maintain accurate and up-to-date patient medical records, ensuring compliance with legal and ethical guidelines.
• Participate in quality improvement initiatives and ongoing professional development to stay current with best practices in cardiometabolic care.
• Adhere to all relevant laws, regulations, and industry standards, including patient privacy and telehealth regulations.

Requirements

• Skilled Nurse Practitioner with a thorough understanding of cardiometabolic condition management, including disease processes and treatment modalities.
