Requirements

Candidates must possess a Certified Professional Coder (CPC) certification and a Certified Risk Adjustment Coder (CRC) certification, along with a minimum of three years of experience in Risk Adjustment HCC coding or abstraction and at least two years of experience with ICD-10 coding and chart review auditing. A comprehensive understanding of HCC coding rules and regulations is required, as well as proficiency in Excel functions and preferred proficiency in the Epic system.

Responsibilities

The Clinical Documentation Reviewer will provide real-time documentation support to healthcare providers before and after patient encounters, ensuring accurate and complete documentation of patient diagnoses, treatments, and procedures. They will clarify and query providers for additional information, work closely with physicians and APPs to improve documentation practices, review patient charts to identify gaps in documentation and coding, conduct training sessions for providers on documentation best practices, and stay updated on coding guidelines, regulatory requirements, and clinical trends while ensuring compliance with ICD-10 and CMS coding guidelines.
