Responsibilities

The Virtual Intake Registered Nurse will guide the clinical enrollment process for members, determining placement in appropriate programs based on eligibility requirements. They will partner with care teams to activate members in Twin programs and support the identification of member goals, health priorities, and learning opportunities. This role involves utilizing motivational interviewing techniques and designing interventions to build member engagement and improve health outcomes. The nurse will also leverage data to observe trends and develop corrective action plans to support member transformation.
