  • Utilize motivational interviewing techniques to build engagement and improved health outcomes
  • Design interventions with members to support improved health outcomes
  • Identify member goals, health priorities, and learning opportunities
  • Support a dynamic care team in identifying member needs and developing care plans
  • Work efficiently and effectively with competing priorities
  • Leverage data to observe trends and develop corrective action plans to support member health outcomes
