Responsibilities

Oversee and guide the clinical enrollment process for members
Determine placement in the most appropriate member program according to eligibility requirements
Activate members in Twin programs
Partner across care teams to support member goals and health priorities
Utilize motivational interviewing techniques to build engagement and improved health outcomes
Design interventions with members to support improved health outcomes
Identify member goals, health priorities, and learning opportunities
Support a dynamic care team in identifying member needs and developing care plans
Work efficiently and effectively with competing priorities
Leverage data to observe trends and develop corrective action plans to support member health outcomes
