We are looking for an independently licensed mental health professional (LCSW, LMFT, LPC required) to join our Care Navigation team. This role will focus on supporting members seeking help for post-traumatic stress disorder (PTSD) and complex trauma.

Reporting to: Clinical Lead of the Trauma Specialty Care Program.

Shift: Monday-Friday, 9 am - 5 pm EST.


Responsibilities

  • Member Support & Referral:
    • Conduct telephonic screening, brief intervention, and referral of members to trauma specialty treatment providers.
    • Utilize active listening, psychoeducation, and motivational interviewing skills to understand member needs and support goal setting.
    • Apply Judith Herman’s phase-based trauma treatment model for stabilization support.
    • Assist members in engaging in care using motivational interviewing.
    • Identify appropriate levels of care, facilitate referrals, and coordinate care with Spring Health providers.
    • Provide consultation on mental health concerns and family/relationship issues as needed.
    • Offer in-the-moment support and safety planning for members experiencing safety risks or crises.
  • Documentation & Coordination:
    • Utilize written communication skills for member and provider interactions via email.
    • Document information, maintain accurate records, perform follow-ups, and track progress within various systems, including an EHR, within required timeframes.
    • Identify and problem-solve barriers or disruptions to care.
    • Collaborate with multidisciplinary teams (care support, provider operations, benefits/billing) for a seamless member experience.
  • Team Collaboration & Development:
    • Assist other specialty care teams (SUD, Mood and Anxiety, Eating Disorders) as needed.
    • Run a weekly provider consultation group focused on trauma-focused evidence-based treatments (EBTs).
    • Participate in staff meetings, case consultations, and trainings.
    • Assist with onboarding and mentoring new team members.
    • Adhere to Spring Health policies and maintain all confidentiality, compliance, and ethical standards.

What Success Looks Like

  • Maintain high member satisfaction, as measured by survey responses.
  • Retain members throughout their course of care through consistent outreach and contact.
  • Triage and enroll members into appropriate levels of care based on their presenting needs.

Requirements

  • Independently licensed mental health professional (LCSW, LMFT, LPC required).
  • Experience supporting members with PTSD and complex trauma.
  • Proficiency in active listening, psychoeducation, and motivational interviewing.
  • Familiarity with Judith Herman’s phase-based trauma treatment model.
  • Strong written and verbal communication skills.
  • Experience with electronic health records (EHR) and documentation.
  • Ability to work independently and as part of a multidisciplinary team.
