Requirements

Candidates must be independently licensed mental health professionals (LCSW, LMFT, LPC) to support members with PTSD and complex trauma. Experience with Judith Herman's phase-based trauma treatment model and motivational interviewing is required. Strong written communication skills for interacting with members and providers via email are also necessary.

Responsibilities

The Trauma Clinical Care Navigator will provide initial assessment, support, referral, and ongoing care coordination for members within Spring Health's Trauma program. Responsibilities include telephonic screening, brief intervention, referral to trauma specialty providers, applying active listening and psychoeducation, utilizing phase-based trauma treatment models for stabilization support, identifying appropriate levels of care, coordinating care with providers, providing consultation on mental health and family issues, offering in-the-moment support and safety planning, documenting information accurately in an EHR, problem-solving barriers to care, interacting with multi-disciplinary teams, assisting other specialty care teams, running weekly provider consultation groups, participating in staff meetings and trainings, and mentoring new team members. Success is measured by maintaining high member satisfaction and retaining members throughout their course of care.
