Requirements

Candidates must possess a Bachelor of Science in Nursing (BSN), an active Compact RN license, and a California Nursing License. A minimum of 3 years of nursing experience is required, with at least 2 years each in care/case/disease management, labor and delivery, and infertility. Current CCM certification or eligibility for the August 2025 exam is mandatory. The role requires experience in a health plan, navigator, or TPA environment, comfort with technology and EMR platforms, strong empathy, and Spanish speaking skills are desirable. Candidates must reside in a compact NLC state and be willing to obtain licenses in multiple states.

Responsibilities

The Nurse Care Manager will provide telephonic, patient-centered virtual care management for members navigating infertility and maternity journeys. Responsibilities include creating personalized care plans, educating members, advocating for their needs, and helping them navigate complex medical conditions, treatment pathways, benefits, and the healthcare system. The role involves partnering with members' providers, providing longitudinal follow-up care, and ensuring adherence to security and HIPAA regulations. Additionally, the nurse will translate medical information into patient-friendly language and collaborate with diverse clinical and non-clinical teams to deliver high-quality care.
