Requirements

  • Master’s degree in Social Work with an active clinical license in any US state - LCSW, LMSW, or comparable state clinical license
  • Willingness to gain additional state licenses
  • Minimum of 5 years experience in the behavioral health or care management fields
  • Competent performing comprehensive psychosocial assessments over video and phone
  • Able to independently manage a fully remote work schedule, M-F, 8:30am-5:00pm CST or MST

Responsibilities

  • Performing comprehensive psychosocial assessments via phone/video with members and caregivers
  • Conduct clinical social work interventions to maximize the effectiveness of the program including: brief therapeutic interventions, assistance in navigating family/support issues, assessing and intervening on social determinants of health, completing safety assessments
  • Interdisciplinary collaboration with teammates (including clinicians, nurses, and care coordinators) to ensure cohesive, patient-centered care for high-needs patients
  • Ability to clinically identify and escalate complex members to the BH Medical Director for consultation
  • Actively participate in team huddles, panel reviews, interdisciplinary team rounds, and program/role growth and development
  • Document all clinical interactions in the electronic health record
