Responsibilities

Provide social services to patients and their families/significant others
Provide therapeutic treatment services to individuals, groups, families, and/or significant others
Conduct individual treatment sessions with patients as needed
Provide crisis intervention services and assess for safety daily
Interact with patients, family members, and referents with respect and dignity
Participate in psychoeducational support sessions
Provide case management and therapeutic interventions to all assigned patients
Serve as patient advocate when a patient has a grievance
Implement goals and interventions as specified on the Care Plan
Ensure goals are SMART (Specific, Measurable, Attainable, Relevant, and Timely)
Document progress according to the treatment plan
Complete discharge documentation and discharge summary
Identify patient’s presenting problem, progress, current status, and treatment recommendations
Contact outpatient providers, schools, and other stakeholders to discuss aftercare
Report and document suspected cases of physical, sexual abuse, and/or neglect
Implement understanding of System policies and procedures
Evaluate the outcome of interdisciplinary interventions and modify the Care Plan accordingly
Assist with the development and evaluation of treatment needs
Participate and/or facilitate interdisciplinary staff meetings
