Requirements

  • Experience in social services and/or therapeutic treatment
  • Understanding of cognitive behavioral therapies
  • Ability to provide crisis intervention services
  • Strong interpersonal and communication skills
  • Knowledge of System policies and procedures
  • Mandatory Reporting knowledge (state-specific)

Responsibilities

  • Provide social services to patients and their families/significant others
  • Provide therapeutic treatment services to individuals, groups, families, and/or significant others
  • Conduct individual treatment sessions with patients as needed
  • Provide crisis intervention services and assess for safety daily
  • Interact with patients, family members, and referents with respect and dignity
  • Participate in psychoeducational support sessions
  • Provide case management and therapeutic interventions to all assigned patients
  • Serve as patient advocate when a patient has a grievance
  • Implement goals and interventions as specified on the Care Plan
  • Ensure goals are SMART (Specific, Measurable, Attainable, Relevant, and Timely)
  • Document progress according to the treatment plan
  • Complete discharge documentation and discharge summary
  • Identify patient’s presenting problem, progress, current status, and treatment recommendations
  • Contact outpatient providers, schools, and other stakeholders to discuss aftercare
  • Report and document suspected cases of physical, sexual abuse, and/or neglect
  • Implement understanding of System policies and procedures
  • Evaluate the outcome of interdisciplinary interventions and modify the Care Plan accordingly
  • Assist with the development and evaluation of treatment needs
  • Participate and/or facilitate interdisciplinary staff meetings
