Responsibilities

  • Lead a team of TOC coordinators in serving as advocates for members
  • Manage a team of TOC coordinators and guide their work to take care of members during the transition of care episode
  • Connect with members, enable ancillary services, and follow-up communication
  • Participate in rounds/huddles and provide support to the team
  • Develop new workflows and job aids
  • Coach and manage performance of TOC coordinators
  • Attend and/or facilitate regular meetings with team members and leadership
  • Collaborate with leadership to develop the TOC program
  • Join cross-functional meetings to represent TOC coordination and share updates
  • Assist with monitoring team attendance and adherence to meetings/forums
  • Participate in recruiting and hiring process of new TOC Coordinators
  • Partner with the TOC trainer and DM education team in onboarding new TOC Coordinators
  • Manage TOC coordinator performance across key performance indicators
  • Provide oversight and support to TOC Coordinators
  • Process improvement: assist in design and implementation of new workflows, including job aid creation and facilitating trainings
  • Meet with Senior Manager regularly to discuss team performance and make recommendations for improvement
