Requirements

Master’s degree in social work
Active license with the willingness to gain additional state licenses as identified by program need
Minimum of 3 years working in medical social work, case management, or care coordination
Embrace, crave, and be comfortable with new and ever-changing technologies
Strong interpersonal and communication skills
Comfortable working with patients over video and phone
Bilingual fluency (written and spoken) in Spanish, Creole, or Vietnamese
Experience with the Medicare Advantage population and health insurance systems

Salary range: $70,000 - $85,000 annually

Responsibilities

Conduct comprehensive social needs screenings and develop individualized care plans that align community, clinical, and financial resources
Interdisciplinary Collaboration: Work seamlessly with clinicians, case managers, and community health workers to ensure cohesive, patient-centered care
Psychosocial Screening & Planning: Conduct comprehensive social needs screenings and develop individualized care plans that align community, clinical, and financial resources
Complex Family Support: Assist in navigating complex family/support issues, recognizing caregiver burnout, and facilitating access to needed emotional and practical resources
Advanced Care Planning: Facilitate sensitive discussions regarding advanced care planning, end-of-life planning, and palliative or hospice care transitions when appropriate
Behavioral Health Coordination: Assess behavioral health needs and coordinate care with the Devoted Medical Behavioral Health Team
Regulatory Compliance: Uphold compliance standards, including investigating and reporting concerns for patient abuse, neglect, or exploitation
