Solace is a healthcare advocacy marketplace connecting patients and families to experts who help them understand and take charge of their personal health. This role focuses on supporting Medicare patients throughout their healthcare journey, addressing their needs and Social Determinants of Health (SDOH).

Requirements

• RN license in good standing
• Deep understanding of Social Determinants of Health (SDOH)
• Experience working with diverse patient populations
• Strong clinical skills
• Exceptional organizational abilities
• Ability to balance multiple tasks and work under pressure
• Technical savvy and willingness to learn new systems and software
• Empathy for people and a willingness to advocate for them

Responsibilities

• Build strong, trusting relationships with Medicare patients.
• Identify and prioritize Medicare patients’ needs.
• Develop comprehensive patient care plans that holistically address social determinants of health (e.g., food resources, transportation access, support at home).
• Navigate patients through difficult and complex health concerns.
• Maintain a streamlined care continuum for patients.
• Learn and utilize Solace systems, tools, technology, partners, and expectations.
• Provide fearless feedback.
