Requirements

Candidates must be a graduate of an accredited school of nursing with an R.N. license, and possess at least 3 years of experience as a Registered Nurse. They must be able to commute to Prince George’s County, Montgomery, Baltimore City, and Baltimore.

Responsibilities

The Resource RN will support the care team by serving as a bridge between the care management team, hub-based clinical team, and members, providing nursing support and consultation. They will collaborate with care team members for case review and care planning, conduct medication reconciliation and education, perform in-home visits and assessments, utilize preventive health screening tools, and address quality gaps. Furthermore, the Resource RN will facilitate follow-ups, triage referral needs, schedule training, and support operational efficiency by utilizing various platforms for data collection and communication.
