Active LPN or RN license in the state of practice
Ability to practice under supervision (for LPN: RN, APRN, physician, or other provider)

Responsibilities

Provides resident care by reviewing physician prescriptions, communicating nursing care plans, coordinating with healthcare providers, and offering physical, emotional, psychological, and spiritual support to residents, families, and friends
Develops nursing care plans by obtaining medical histories, examining residents, documenting physical condition, psychological status, and social factors, conferring with family and friends, developing nursing diagnoses, establishing care objectives, and defining interventions, schedules, and timetables
Teaches healthcare requirements by establishing resident care goals, instructing residents, families, and friends on conditions, medications, and self-care skills, answering questions, and providing information and resources
Evaluates resident care by adhering to therapeutic standards, measuring health outcomes, reviewing records, noting changing conditions and needs, re-evaluating care objectives, and modifying nursing care plans
Maintains safe, secure, and healthy resident care environment by following infection-control policies, medication administration and storage procedures, controlled substance regulations, organization standards, policies, and legal regulations; alerting others to potential concerns
Keeps resident care equipment operating by following instructions, troubleshooting breakdowns, maintaining supplies, performing preventive maintenance, and calling for repairs
Keeps resident care supplies ready by checking stock, anticipating needs, placing and expediting orders, verifying receipt, and stocking items
Organizes and directs aspects of care for which accountable, communicates and coordinates care with nursing diagnoses and all health team members
