Registered Nurse (RN) license
Meets RN CAP 2 criteria or advancement from RN CAP 1
Ability to perform physical demands including heavy lifting, moving patients, and assisting with ambulation
Completion of CAP requirements to maintain or advance

Responsibilities

Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document in medical record, adjust treatment plans, and notify Supervisor or physician as needed
Assess, collaborate, and document patient/family’s basic learning needs; provide initial and ongoing education
Direct and provide, in collaboration with patient, home care partner, and staff, safe and effective delivery of dialysis therapies
Administer medications as prescribed or per approved algorithms, documenting justification and effectiveness
Initiate or assist with emergency response measures
Serve as resource, leader, coach, mentor, and role model for employees by setting examples of behavior, work habits, and attitudes
Ensure correct laboratory collection, processing, shipping; reschedule missed or insufficient collections
Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with Interdisciplinary Team
Ensure patient awareness of transplant and treatment modality options
Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
Perform assessment of Home Dialysis candidate’s home environment, partner/family readiness, and ability to perform treatments
Train home dialysis patients and/or Home Partners on safe, effective operation and maintenance of Home Dialysis equipment and supplies via formal training program
Participate in education and quality improvement projects at facility and area level
May serve as Preceptor to new employees
Perform all other duties as assigned by Supervisor
Perform all essential functions under direction of Supervisor and with guidance from Educator, Preceptor, or in collaboration with another RN, in accordance with policy, procedures, standards, and regulations
