  • Registered Dietitian licensure
  • Knowledge of diet, kidney disease, and its impact on treatment processes
  • Ability to assess patient's nutritional status, knowledge, and learning needs
  • Proficiency in calculating diet prescriptions according to standards of practice
  • Skills in nutritional assessment, counseling, and education for patients, families, and caregivers
  • Competence in reviewing and interpreting nutrition-related lab results (e.g., albumin, body weight, Kt/V, mineral metabolism, anemia)
  • Ability to collaborate with interdisciplinary healthcare teams, including physicians, case managers, and facility staff
  • Familiarity with Federal and state regulations, FMCNA guidelines, QEP, QAI, and process improvement activities
  • Experience with documentation in medical records, progress notes, and plan of care development
  • Capability to identify malnutrition risks and recommend interventions

Responsibilities

  • Assess patient’s knowledge of diet and kidney disease; provide tailored education on nutritional status, adequacy measures, fluid status, and lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments
  • Develop written, individualized comprehensive plans of care with the interdisciplinary team
  • Calculate diet prescriptions and provide ongoing nutrition assessment and counseling
  • Monitor patient adherence and response to nutrition therapy; adjust as needed and document outcomes
  • Review monthly albumin levels and body weight; implement plans of care with patient/family involvement
  • Collaborate on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Maintain accurate documentation in medical records per FMCNA policies and regulations
  • Identify malnourished or at-risk patients and work with team on interventions
  • Report on nutrition QAI results and participate in interdisciplinary QAI programs
  • Coordinate Pre-ESRD education and refer patients for nutritional supplements as appropriate
  • Review nutrition-related lab results, counsel patients/caregivers, recommend treatment changes, and communicate with physicians, staff, and external facilities
  • Participate in staff meetings and quality improvement activities
