Requirements

Candidates must be a graduate of an accredited School of Nursing with a current, unencumbered RN license. A minimum of 1 year of home health experience is required, along with a working knowledge of Title XXII, Medicare Home Health Conditions of Participation, and OASIS. Excellent written and oral communication skills, the ability to handle stressful situations and deadlines, and strong computer skills are essential. Proficiency with Home Care Home Base software is highly preferred. The ideal candidate is a self-starter, goal-oriented, capable of working independently, and able to communicate effectively with agency staff, upper management, and the healthcare team.

Responsibilities

The Quality Care Manager is responsible for reviewing patient assessments to ensure appropriate and effective care in compliance with agency policies and regulatory requirements. This includes communicating effectively with the healthcare team and leadership, reviewing documentation for accuracy and completeness to support skilled need and medical necessity, and reinforcing agency processes with clinicians. The QCM will ensure visit utilization is supported by patient assessments, review additional orders for appropriateness, confirm clinical documentation supports recertification/discharge decisions, and approve the final Plan of Care (485) timely. They will also review all OASIS assessments for inconsistencies and lock them promptly. Any identified trends or patterns of inaccuracy will be communicated to the Quality Director to aid in performance improvement and education plans.
