State licenses in WI, MN, and IA
3+ years of post-residency work experience
Comfortable practicing the full spectrum of comprehensive care, including mental health
Eligible for IMLC

Responsibilities

Provide on-demand Acute Care and scheduled Primary Care
Educate patients on appropriate treatments and care plans for their health needs
Manage patient follow-ups for chronic care programs
Co-manage patients with external partners within our healthcare delivery model
Prescribe medication and refills as appropriate
Drive high-quality care with a focus on patient outcomes
Elevate remote care by providing compassionate and meaningful patient care
Collaborate with Clinical Team Members to build clinical guidelines and policies
Perform against challenging goals with a best-in-class team to deliver Quality Care-Efficiently
Adhere specifically to all company policies and procedures, Federal and State regulations and laws
Display dedication to the position responsibilities to achieve assigned goals and objectives
Represent K Health and our partners in a professional manner and appearance at all times
