Candidates must possess a Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy degree, along with a current, active pharmacist license in good standing in [Insert State]. Strong clinical knowledge and understanding of pharmacotherapy principles are required, as well as excellent communication and organizational skills.
The Pharmacist will review and verify new patient orders and accompanying documentation, collaborate with physicians and healthcare professionals to ensure medication safety and effectiveness, provide clinical consultations and patient counseling on drug therapies, monitor ongoing medication therapy for appropriateness and adherence, manage medication supplies, prepare and dispense medications according to established procedures and standards, and maintain thorough and accurate documentation of all clinical activities.
