Responsibilities

Helpen bij de dagelijkse zorg en verzorging van cliënten met NAH
Ondersteunen bij structuur, rust en overzicht
Samen met de cliënt en netwerk een persoonlijk plan opstellen dat aansluit bij wensen en mogelijkheden
Een luisterend oor bieden en goede gesprekken voeren
Samenwerken met collega's, gedragsdeskundigen en familie om verbeteringen te bespreken en doelen op te stellen
