Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and corporation, state, and federal regulations
  • Knowledge of dialysis therapy, including setup and operation of hemodialysis machines
  • Ability to perform vascular access evaluation, cannulation, and heparin administration as delegated or allowed by state law
  • Skills in obtaining vital signs, weights, and performing pre/post dialysis evaluations
  • Capability to initiate, monitor, and discontinue dialysis treatments per prescribed orders
  • Proficiency in calculating and entering dialysis machine programming (e.g., UF goal, treatment time, UF modeling)
  • Ability to enable safety alarms (e.g., Narrow Venous Limits at 160)
  • Accurate and timely documentation on Hemodialysis Treatment Sheet and progress notes
  • Observation and machine safety checks per facility policy
  • Ability to obtain laboratory testing and prepare specimens
  • Knowledge of cleaning protocols for blood spills and maintaining a safe, clean environment
  • Efficient use of supplies and equipment to prevent wastage
  • Support for Quality Enhancement Program (QEP), CQI activities, patient satisfaction, and process improvement

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease process/access
  • Welcome patients and inquire about wellbeing
  • Evaluate vascular access for patency, cannulate, and administer heparin as allowed
  • Obtain pre and post treatment vital signs, weight, and vascular access evaluation
  • Initiate dialysis per orders (QB, QD), evaluate intradialytic problems, and intervene as prescribed
  • Monitor patient response to therapy, obtain vital signs before reinfusion, and discontinue treatment
  • Evaluate patient prior to termination of venous access (standing/sitting BP), achieve hemostasis, and apply dressings
  • Evaluate patient prior to discharge and record pre/post evaluations
  • Calculate and enter patient-specific dialysis programming for UF goal, time, and modeling
  • Ensure safety alarms are enabled
  • Record vital signs and treatment parameters accurately on Hemodialysis Treatment Sheet
  • Document patient treatment information and report significant changes to RN/supervisor
  • Observe patient and perform/document machine safety checks; report unusual findings
  • Monitor vascular access during treatment, intervene as needed, and report findings
  • Obtain prescribed lab testing and prepare specimens
  • Clean blood spills immediately per policy
  • Prepare, organize, and utilize supplies/equipment efficiently
  • Monitor patients performing self-care under RN supervision
  • Assist in training direct patient care staff
