Requirements

  • Works full-time days (MTTF schedule) as Home Therapy PCT II

Responsibilities

  • Provides direct patient care for assigned patients in skilled nursing homes under RN supervision, in accordance with physician orders, state nurse practice laws, FMCNA policies, procedures, training, and regulations
  • Promptly arrives at patient's nursing location and inquires about wellbeing since last treatment
  • Prepares and tests water and dialysis equipment per policies, documenting all required elements
  • Actively participates in process improvement activities to support FMC Quality Enhancement Goals (QEP), QAI, and CQI, including patient satisfaction
  • Records pre- and post-treatment observations, weight, and vital signs
  • Calculates and enters dialysis machine programming (UF goal, treatment time, Na modeling, UF modeling) for RN review and approval
  • Performs vascular access cannulation, observing for patency, under RN supervision and as allowed by state law
  • Administers Heparin per physician orders under RN supervision and as allowed by state law
  • Monitors and assists with treatment administration (dialysis time, QB, QD) per prescribed orders
  • Observes patient and conducts machine safety checks per policy; ensures safety alarms (air detector, NVL 160) are enabled
  • Monitors vascular access (arterial/venous pressures) during treatment
  • Records accurate, timely vital signs and treatment parameters on Hemodialysis Treatment Sheet
  • Inquires about intra-dialytic problems and monitors patient response to therapy, providing interventions as prescribed
  • Obtains vital signs prior to treatment termination, prepares for discontinuation, and documents
  • Discontinues dialysis treatment per procedures; observes patient, access condition, and blood pressure (standing/sitting), obtains homeostasis
  • Empowers, engages, and trains dialysis patients on peritoneal and home hemodialysis using state-of-the-art programs/equipment
  • Primarily works in clinic setting; conducts initial home visits with another staff member
  • Functions as onsite Patient Educator and Case Manager with various shifts/work hours
  • Supports FMCNA mission, vision, core values, customer service philosophy, and Compliance Program
