Requirements

  • Ability to pass Ishihara's Color Blindness test (reasonable accommodations considered)
  • Frequent, prolonged periods in front of a computer and phone work
  • Frequent, prolonged standing and ability to bend over
  • Push/pull equipment up to 15 pounds
  • Lift/lower solutions up to 30 lbs. frequently and up to 40 lbs. occasionally (as high as 5 feet, with two-person assist)
  • Work in health care facility environment with potential exposure to infectious/contagious diseases, moderate noise, and air temperature control

Responsibilities

  • Assist with administrative duties related to patient appointments
  • Greet new and existing patients and notify Patient Care Team of arrival
  • Schedule appointments (including for access placement/removal) and make reminder calls
  • Answer home therapy program calls and follow up on voicemails with care team
  • Coordinate transportation assistance as needed
  • Prepare room/space for patient arrival, ensuring cleanliness and compliance with Infection Control standards
  • Place supply orders and follow up on patient-related orders
  • Prepare items for patients, including education materials and lab supplies
  • Track extension set changes, vaccine due dates, hospitalizations, transplant referrals, and follow-ups
  • Ensure patients’ records are available for review
  • Assist patients to/from waiting area
  • Obtain height, weight, and vital signs
  • Perform venipunctures for laboratory testing and process lab samples
  • Follow up on referrals and additional appointments
  • Document patient information
  • Maintain record and supply rooms: file/upload records, store documents, track transfer set changes, Kt/Vs, CIA, POC home visits, vaccines, hospitalizations, infections, monthly education, MD/employee certifications/licensures/annual skills, medical record retention checklist, transplantation-lab collecting/tracking
  • Check for expired medications, lab tubes, and supplies
  • Count and order nonprescription supplies; assist in ordering supplies including medications
  • Review and comply with Code of Business Conduct, company policies, and laws/regulations
  • Keep assigned areas cleaned and stocked
  • Assure confidentiality of patient information and medical records
  • Participate as part of Care Team Hub and other assigned duties
