Requirements

  • Unrestricted RN licensure, compact preferred
  • Ability to get licensed in additional states if requested within 90 days of hire date
  • A minimum of 5 years of RN experience, with at least 2 years in hospice or palliative care
  • CHPN® credential, or ability/willingness to obtain within first six months of employment
  • Willingness to flex between time zones
  • Comfortable with technology and a fast-paced environment
  • Self-starter with ability to succeed in a work-from-home environment
  • Strong interpersonal and communication skills
  • Active Basic Life Support (BLS) certification
  • Preferred
  • ELNEC® or EPEC® training
  • Bilingual in English and Creole, Spanish, or Vietnamese
  • Prior ICU, oncology or geriatrics nursing experience
  • Prior inpatient nursing experience
  • Prior program development experience

Responsibilities

  • Engage with patients telephonically and/or via video to provide home-based palliative care rooted in compassionate, evidence-based clinical practice
  • Perform essential care coordination and case management tasks (e.g. following up on DME orders, collaborating with home health workers, communicating with PCP, etc.)
  • Participate in daily team huddles and weekly palliative rounds with interdisciplinary team
  • Maintain patient panel; conduct routine check-ins based on risk stratification or change in health status; refer to palliative social worker and NP as needed
  • Collaborate with other Devoted Medical teams as needed to provide the highest quality of integrated care for our shared patients
  • Assist with triaging and responding to incoming calls from palliative patients
  • Support palliative education initiatives for Devoted Medical employees in collaboration with palliative teammates
  • Work closely with your palliative care teammates to support, care for and grow the program, patients, and each other
  • Shift flexibility to cover members based in PST as needed
