Requirements

  • Registered Nurse (RN) license in good standing
  • RN CAP 2 designation or advanced from RN CAP 1

Responsibilities

  • Performs ongoing assessment, planning, intervention, and evaluation of patients undergoing hemodialysis treatment
  • Documents patient data and makes adjustments to the treatment plan as needed
  • Notifies Team Leader, Charge Nurse, Supervisor, or Physician of any concerns
  • Collects and analyzes patient data pre-, during-, and post-hemodialysis
  • Assesses patient/family’s basic learning needs and provides initial and ongoing education
  • Directs and provides daily hemodialysis therapy to assigned patients in collaboration with other staff
  • Administers medications as prescribed or in accordance with approved algorithms, documenting justification and effectiveness
  • Initiates or assists with emergency response measures
  • Serves as a resource, leader, coach, mentor, and role model for new and incumbent employees
  • Ensures correct laboratory collection, processing, and shipping procedures are performed
  • Collaborates with the Interdisciplinary Team to update nursing assessments and plans of care
  • Ensures patient awareness related to transplant and treatment modality options
  • Participates in education and quality improvement projects
  • May serve as a Preceptor to new employees
