Registered Nurse (RN) license in good standing
RN CAP 2 designation or advanced from RN CAP 1

Responsibilities

Performs ongoing assessment, planning, intervention, and evaluation of patients undergoing hemodialysis treatment
Documents patient data and makes adjustments to the treatment plan as needed
Notifies Team Leader, Charge Nurse, Supervisor, or Physician of any concerns
Collects and analyzes patient data pre-, during-, and post-hemodialysis
Assesses patient/family’s basic learning needs and provides initial and ongoing education
Directs and provides daily hemodialysis therapy to assigned patients in collaboration with other staff
Administers medications as prescribed or in accordance with approved algorithms, documenting justification and effectiveness
Initiates or assists with emergency response measures
Serves as a resource, leader, coach, mentor, and role model for new and incumbent employees
Ensures correct laboratory collection, processing, and shipping procedures are performed
Collaborates with the Interdisciplinary Team to update nursing assessments and plans of care
Ensures patient awareness related to transplant and treatment modality options
Participates in education and quality improvement projects
May serve as a Preceptor to new employees
