Requirements

  • Registered Nurse (RN) license in good standing
  • Completion of the Clinical Advancement Program (CAP) requirements
  • Ability to perform ongoing, systematic collection and analysis of patient data
  • Knowledge of hemodialysis treatment procedures
  • Ability to assess patient/family learning needs and provide education
  • Ability to initiate or assist with emergency response measures

Responsibilities

  • Performs ongoing assessment, planning, intervention, and evaluation of patient care under the direction of a Supervisor and Clinical Educator
  • Collects and analyzes patient data pre-, during-, and post-hemodialysis treatment, documenting findings and making necessary adjustments to the treatment plan
  • Collaborates with the interdisciplinary team to identify expected outcomes and update the nursing assessment and plan of care
  • Directs and provides hemodialysis therapy to assigned patients, in collaboration with direct and ancillary patient care staff
  • Administers medications as prescribed or in accordance with approved algorithms, documenting medical justification and effectiveness
  • Provides initial and ongoing education to patients and families regarding transplant and treatment modality options
  • Serves as a resource for the healthcare team and participates in staff training and orientation
  • Ensures correct laboratory collection, processing, and shipping procedures are performed
  • Reschedules missed or insufficient laboratory collections
  • Identifies and addresses patient’s basic learning needs
  • Participates in decision-making, teaching, leadership functions, and quality improvement activities
