Requirements

Registered Nurse (RN) license in good standing
Completion of the Clinical Advancement Program (CAP) requirements
Ability to perform ongoing, systematic collection and analysis of patient data
Knowledge of hemodialysis treatment procedures
Ability to assess patient/family learning needs and provide education
Ability to initiate or assist with emergency response measures

Responsibilities

Performs ongoing assessment, planning, intervention, and evaluation of patient care under the direction of a Supervisor and Clinical Educator
Collects and analyzes patient data pre-, during-, and post-hemodialysis treatment, documenting findings and making necessary adjustments to the treatment plan
Collaborates with the interdisciplinary team to identify expected outcomes and update the nursing assessment and plan of care
Directs and provides hemodialysis therapy to assigned patients, in collaboration with direct and ancillary patient care staff
Administers medications as prescribed or in accordance with approved algorithms, documenting medical justification and effectiveness
Provides initial and ongoing education to patients and families regarding transplant and treatment modality options
Serves as a resource for the healthcare team and participates in staff training and orientation
Ensures correct laboratory collection, processing, and shipping procedures are performed
Reschedules missed or insufficient laboratory collections
Identifies and addresses patient’s basic learning needs
Participates in decision-making, teaching, leadership functions, and quality improvement activities
