Entry level designation into the Clinical Advancement Program (CAP)
Accountability and responsibility for provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
Ability to delegate appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
Participation in decision-making, teaching, leadership functions, and quality improvement activities
Completion of CAP requirements to advance
Ability to perform physical demands: heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. (with assistance), lifting chemical/water solutions up to 30 lbs. as high as 5 feet
Tolerance for work environment: health care facility with air temperature control, moderate noise, exposure to infectious/contagious diseases, desk/computer work, patient/staff/physician interaction, possible travel

Responsibilities

Perform all essential functions under direction of Supervisor and with guidance from Clinical Educator, Preceptor, or in collaboration with other RNs
Perform ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment, document in medical record, adjust/modify treatment plan, notify Team Leader/Charge Nurse/Supervisor/Physician as needed
Assess, collaborate, and document patient/family’s basic learning needs; provide initial and ongoing education
Direct and provide, in collaboration with staff, all aspects of daily safe and effective hemodialysis therapy delivery
Administer medications as prescribed or per approved algorithm(s); document justification and effectiveness
Initiate or assist with emergency response measures
Serve as resource for health care team; participate in staff training and orientation of new staff
Ensure correct laboratory collection, processing, shipping; reschedule missed/insufficient collections
Identify expected outcomes; document and update nursing assessment and plan of care via collaboration with Interdisciplinary Team
Ensure patient awareness of transplant and treatment modality options
Perform all other duties as assigned by Supervisor
