Are you passionate about helping patients navigate their medications and improve their health? Join our Clinical Care team at Arine! As a Nurse on our team, you will play a key role in supporting patients post-discharge, enhancing care coordination, and helping them better understand and manage their medications, especially for conditions like heart disease and diabetes.
Using Arine’s innovative platform, you’ll conduct outreach to patients, providers, and pharmacies to perform medication reconciliation, provide education, and ensure adherence to treatment plans. You’ll collect clinical data, such as blood pressure readings, and work closely with pharmacists and other healthcare professionals to identify and resolve medication-related issues that may impact patient outcomes or lead to readmissions.
In this role, you’ll also support quality improvement efforts tied to HEDIS goals, help gather necessary documentation for CMS measures, and contribute to improving operational workflows. Your input will be instrumental in triaging high-risk patients and ensuring continuity of care across multiple settings.
