Candidates must be a graduate of an accredited School of Nursing and hold a current, unencumbered license to practice as a Registered Nurse (RN). A minimum of 1 year of home health experience is required, along with a working knowledge of Title XXII, Medicare Home Health Conditions of Participation, and OASIS. Excellent written and oral communication skills are necessary, as is the ability to handle stressful situations and deadlines. Strong computer and software skills are essential, with knowledge of Home Care Home Base being highly preferred. The ideal candidate is a self-starter, goal-directed, and capable of working independently without on-site supervision. The ability to communicate effectively with agency staff, upper management, and other members of the healthcare team is also required.
The Quality Care Manager is responsible for reviewing patient assessments to ensure appropriate and effective care is provided in compliance with agency policies, procedures, and regulatory requirements. This includes communicating effectively with the healthcare team and leadership, performing reviews of documentation and plans of care for accuracy and completeness, and confirming that all documentation supports skilled need and medical necessity. The QCM will reinforce agency processes by providing feedback to clinicians on documentation deficiencies or OASIS corrections, ensuring visit utilization is supported by patient assessments, and reviewing additional orders for appropriateness. They will confirm that clinical documentation supports Recertification/Discharge decisions, approve the final Plan of Care (485) timely, and review all OASIS assessments for inconsistencies, locking them promptly. Any identified trends or patterns of inaccuracy will be communicated to the Quality Director to aid in the development of performance improvement and education plans.
