Responsibilities

The Mom’s First Maternity, Telephonic Nurse will perform telephonic and face-to-face assessments and evaluations of members’ needs, create member care plans, employ various strategies to manage their health needs, ensure members progress towards desired outcomes through continuous monitoring, identify and resolve barriers to effective care, collaborate with providers and community services, and periodically travel to the Humana Oklahoma office for meetings and training.
