The primary responsibilities include documenting patient encounters (about 80% of the time) by joining providers in the exam room, creating structured notes with history, assessment, plan, physical exam, assigning CPT and ICD-10 codes, preparing after-visit summaries, and clinical documentation improvement (10%) such as reviewing records and using population health tools.
Scribes become experts in value-based care model, documentation and care of chronic conditions, ICD-10 and CPT coding, clinical workflows, preventative care, accurate documentation, population health data, and team-based care through extensive on-the-job training.
The ideal candidate should commit at least 1-2 years to this role.
