Requirements

  • Master of Social Work (MSW) degree (entry level role)
  • Licensed as a Social Worker within the scope of clinical training and state license
  • Knowledge of Social Work Theory of Human Behavior and accepted methods of social work practice
  • Knowledgeable of FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy
  • Current knowledge of local vocational/educational rehabilitation programs
  • Ability to assess psychosocial status, strengths, and areas of need affecting rehabilitation and treatment outcomes
  • Ability to provide supportive counseling, educational and goal-directed counseling (e.g., for transplant seekers), and psychosocial interventions
  • Ability to collaborate with interdisciplinary teams, physicians, nurses, patients, and families
  • Adherence to documentation standards based on MSW interactions and interventions

Responsibilities

  • Assess patients' psychosocial status, strengths, and needs as part of comprehensive patient assessment
  • Participate in care planning with the patient and healthcare team to identify interventions for rehabilitation, treatment goals, and quality of life improvement
  • Utilize FKC patient education programs, social work theory, interventions, and quality of life instruments in assessment and care planning
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life
  • Provide supportive counseling services to patients within scope of training and license
  • Offer educational and goal-directed counseling to transplant-seeking patients
  • Provide information and assist with referrals to community resources (e.g., home health, vocational rehabilitation)
  • Assist patients with access to vocational rehabilitation to maintain, obtain, or pursue employment/education
  • Assess patient awareness of advance directives, facilitate access and discussions with team and family
  • Participate in discussions of patient DNR status with physician, nurse, patient, and family
  • Provide psychosocial support and Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns
  • Participate in monthly Quality review meetings, reporting on indicators like adherence, Missed/Shortened Treatments, Quality of Life Trends, and Service Recovery
  • Assess patient knowledge of kidney disease and barriers to adherence; provide tailored education with patient, family, and team
  • Support FKC Quality Indicators, Outcomes, QAI Activities, and process improvements for patient satisfaction and quality goals
  • Educate and support staff on emotional, psychological, and behavioral impacts of Chronic Kidney Disease
  • Document interactions and interventions provided to patient and/or family
