Requirements

  • Master's degree in Social Work (MSW) - entry level role
  • Current state license as a Master Social Worker (MSW) permitting clinical practice within scope
  • Knowledge of social work theory of human behavior and accepted methods of social work practice
  • Knowledgeable of FMCNA/FKC Social Work Policy and Measuring Patient Physical and Mental Function Policy
  • Current knowledge of local vocational/educational rehabilitation programs
  • Ability to assess psychosocial status, strengths, needs, and barriers affecting rehabilitation and treatment outcomes
  • Ability to provide supportive counseling, educational/goal-directed counseling (e.g., for transplant), and psychosocial interventions within clinical training and state license scope
  • Ability to collaborate with interdisciplinary team, physicians, nurses, patients, and families
  • Ability to document interactions and interventions based on MSW activities

Responsibilities

  • Assess patients' psychosocial status, strengths, and areas of need as part of comprehensive patient assessment
  • Participate in care planning with patient and healthcare team to identify interventions for rehabilitation, treatment goals, and quality of life improvement
  • Utilize FKC patient education programs, social work theory/methods, interventions, and quality of life instruments to address barriers and meet goals
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status/quality of life
  • Provide supportive counseling to patients within scope of clinical training and state license
  • Provide educational and goal-directed counseling to transplant-seeking patients
  • Assist with referrals to community resources (e.g., home health, vocational rehabilitation) for optimal treatment outcomes
  • Assist patients with vocational/educational rehabilitation referrals to maintain, obtain, or pursue employment/education
  • Assess patient awareness of advance directives, facilitate access/discussion of forms, and discuss wishes with team/family
  • Participate in discussions of patient DNR status with physician, nurse, patient, and family to ensure informed decisions
  • Provide psychosocial support/Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns based on acuity
  • Participate in monthly Quality review meetings, reporting on indicators like adherence (Missed/Shortened Treatments), Quality of Life Trends, and Service Recovery
  • Assess patient knowledge of kidney disease for adherence barriers and provide tailored education with patient, family, and team considering learning style/communication needs
  • Inform, educate, and support staff on emotional/psychological/behavioral impact of Chronic Kidney Disease on patients/families
  • Support FKC Quality Indicators, Outcomes, QAI activities, patient satisfaction, quality of life, and process improvement to achieve Quality Goals
  • Document MSW interactions and interventions provided to patient/family
