About the Role

The Manager of Nursing and Community Care is a highly skilled and experienced nurse leader, who is responsible for providing clinical, operational, administrative, and managerial support to the RN team. This role will closely partner with Care Team Leads, and other clinical leaders across the organization to foster operational excellence across multiple interdisciplinary teams.

Responsibilities

  • Oversee and manage a team of registered nurses (RNs), providing clinical, operational, administrative, and managerial support to enhance team effectiveness.
  • Monitor RN team performance and productivity metrics, fostering ongoing professional learning, training, and clinical development.
  • Collaborate with the Executive Director, Market Medical Directors, and Senior Director for Operations and Nursing to establish and implement market-level strategies and tactics.
  • Plan and execute tactical operations in designated coverage areas to achieve market excellence, focusing on communication, organization, resource management, and workforce engagement.
  • Develop, implement, and oversee operational processes and policies to ensure member and client satisfaction, boost efficiency, and enhance quality and productivity.
  • Utilize technology and drive transformational change initiatives to optimize business processes within the area of coverage.
  • Identify operational challenges and opportunities in collaboration with management, supervisors, and staff, fostering innovative solutions to achieve operational excellence.
  • Create a high engagement workforce environment through coaching, setting expectations, conducting performance appraisals, career development, succession planning, corrective actions, and staff recognition.
  • Develop and maintain comprehensive reporting systems, including daily, weekly, monthly, and quarterly reports, alongside key performance indicators to track progress and outcomes effectively.

Requirements

  • You have a current, unrestricted license in the state in which you will be employed to practice with Cityblock.
  • You have 5+ years of experience providing clinical services to Adult and Geriatric individuals with co-occurring chronic medical and behavioral health conditions.
  • 3+ years directly managing a team of RNs (or higher clinical training).
  • Familiarity and willing to travel within the community (home-based member visits) and its healthcare systems (hospitals and rehab centers).
  • Experience as an active participant in continuous quality improvement projects within a provider setting.
  • Possess exceptional triage, coordination and clinical assessment skills.
  • Phenomenal communicator; you approach care interactions with warmth and thoughtfulness.
  • Demonstrated the ability to affect change, and have been effective in helping a member or patient adapt new habits, or change behaviors.
  • Excited about how technology can support your work and help drive the ongoing evaluation toward new and better care.
  • Independent self-starter, a leader, and a strategic thinker who is excited about the big picture of whole community health, and the ongoing evaluation and iteration of our care model.
  • You believe deeply in a localized, community-based care model and are compelled by the mission that drives Cityblock Health.
  • You are willing to work a full-time 40-hour work week, Monday-Friday with Saturday Rotation and participation in the Cityblock on-call schedule.

Compensation and Benefits

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.

The expected salary range for this position is: $104,841 - $119,818 Annually.
