Licensed Practical Nurse (LPN) - active license in the state of practice upon hire, or Certified Medication Aide (CMA) or ability to complete certification courses within 60 days of hire
1-3 years experience in a clinical or long-term care environment (preferred)
Ability to work Night Shift (11:00 p.m. to 7:30 a.m.), rotating weekends/holidays, Part Time (at least 32 hours per pay period)
Visual acuity adequate to perform position duties
Ability to communicate effectively, hear, understand, and distinguish speech and other sounds

Responsibilities

Regularly evaluate residents' physical and mental health, including vital signs, cognitive function, and overall well-being
Accurately dispense and monitor medications according to physician orders
Maintain open communication with residents, families, physicians, and other healthcare team members for coordinated care
Observe and promptly report significant changes in resident health status to appropriate healthcare providers
Assess pain levels and administer pain medication as prescribed
Provide emotional support and address psychosocial needs of residents
Maintain accurate and detailed resident records, including progress notes and medication administration logs
Recognize subtle changes in resident behavior or health status due to chronic conditions
Collaborate with Certified Nursing Assistants (CNAs) and other healthcare professionals to deliver comprehensive care
Comply with all aspects of resident rights, including confidentiality and privacy
Lead by example, following Freeman Regional Health Services CARE standards
