As an independent lease holder, you keep 100% of exam and professional fees, with the ability to set your own fee schedule. You pay fair market value for optional equipment, instrumentation, and administrative or Opt-Tech support services if desired.
This is an in-person role leasing space inside Sam's Club locations. You have full autonomy to establish your days of practice and hours of operation.
You need a Doctor of Optometry (OD) from an accredited school with a valid state optometry license, or an Ophthalmologist (MD or DO) with a valid state ophthalmology license. Additional requirements include a General Liability Certificate, Professional Liability Insurance Certificate, and W-9.
This is not an employment position with Walmart; it is an independent lease agreement where you operate your own practice inside Sam's Club.
You control your patient schedule, set your fee schedule, establish your days and hours, and retain 100% of professional fees.
