Multi-state licenses (preferred)
Able to cover the following primary locations: Vancouver and surrounding area

Responsibilities

Provide In-Home Health Evaluations, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
Conduct visits lasting about 45 minutes using an iPad and intuitive clinical workflow
Bring heart into every visit and build trusted relationships to improve health outcomes
Be flexible to travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
