Multi-state licenses (preferred)
Able to cover the following primary locations: Hartford, CT

Responsibilities

Provide In-Home Health Evaluations in patients' homes (45-minute visits including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered, such as diabetic eye exams, spirometry, etc.)
Use an iPad and intuitive clinical workflow for each evaluation
Do not prescribe medicine, order lab tests, or alter people’s current treatment regimen
Bring heart into every visit and build trusted relationships to improve health outcomes
Be flexible to travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening, etc.)
