Multi-state licenses (preferred)
Able to cover primary locations: Seattle, WA and surrounding area
Able to comfortably and safely perform duties in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
Flexible to travel locally and within licensed states, as needed

Responsibilities

Conduct in-home health evaluations lasting about 45 minutes, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
Use an iPad and intuitive clinical workflow for each evaluation
Bring empathy and build trusted relationships during one-on-one visits to improve health outcomes
Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
Work flexibly as a clinician colleague fulfilling the specified role within a national network
