Responsibilities

Provide In-Home Health Evaluations, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
Conduct visits lasting about 45 minutes using an iPad and intuitive clinical workflow
Bring heart into every visit and build trusted relationships to improve health outcomes
Be flexible to travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
Work comfortably and safely in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
