Responsibilities

Provide In-Home Health Evaluations, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
Bring heart into every visit, building trusted relationships to improve health outcomes
Use an iPad and intuitive clinical workflow for each evaluation
Be flexible to travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
Comfortably and safely perform duties in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
