Active, unrestricted license(s) as Nurse Practitioner or Physician Assistant in coverage area(s) (Erie County/Buffalo, NY and surrounding area)
Board certification
Multi-state licenses (preferred)
Able to cover primary locations: Erie County/Buffalo, New York and surrounding area
Able to comfortably and safely perform duties in diverse home environments (e.g., with household pets, secondhand smoke, airborne allergens or irritants)
Flexible to travel locally and within licensed states, as needed

Responsibilities

Conduct in-home health evaluations (approx. 45 minutes each), including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry, bone density screening)
Use iPad and intuitive clinical workflow for evaluations
Bring empathy and build trusted relationships during visits to improve health outcomes
Conduct virtual visits as needed
Provide all Signify Health services, including Diagnostic & Preventive Services products
Engage one-on-one, answer health questions, and connect people to appropriate care without prescribing medicine, ordering lab tests, or altering treatment regimens
