Candidates must reside in the State of Oklahoma, with the ability for occasional travel to office for staff meetings, training, etc. They should have at least 2+ years of prior experience working with community housing resources, community health agencies/social service agencies (Area Agency on Aging, DME providers, Meal on Wheels etc.). Intermediate to Advanced knowledge of Microsoft Office Suite, including Word, Excel, PowerPoint, and Access is required, along with exceptional communication and interpersonal skills. The ability to multi-task and work in a fast-paced environment is also necessary, as well as decision-making skills regarding work approach and priorities.
The Housing Specialist will find community-based support to meet enrollees’ housing, transportation, food insecurity, education, and employment needs, in addition to other SDOH needs. They will support Care Managers and Community Health Workers to find appropriate community housing resources and address additional SDOH needs. The role involves liaising among Humana Care Managers, providers, and community-based organizations to coordinate referrals for enrollees to community-based services and programs, fostering integrated efforts. The Housing Specialist will handle calls from Enrollee Services, providing initial resource assistance and referring to Care Management when appropriate. They will assist with health screening process, conduct surveys, and offer Care Management services as needed. Serving as a key knowledge source for community services and information for enrollees and Humana associates is also part of the responsibilities, along with implementing community assessments to identify resource gaps, monitoring enrollee compliance with care plans, and providing motivational interviewing. The role includes attending community events, building relationships with providers and resources, and supporting enrollee self-management skills through education and coaching.
